چکیده

Hepatic cirrhosis with portal hypertension and gastroesophageal hemorrhage is a disease complex that continues to be treated by surgical portasystemic shunts. Whether or not a reduction or diversion of portal blood flow to the liver adversely affects the ability of the liver to maintain fuel homeostasis via gluconeogenesis, glycogenolysis, and ketogenesis is unknown. 11 patients with biopsy-proven severe hepatic cirrhosis were studied before and after distal splenorenal or mesocaval shunts. Hepatic, portal, and renal blood flow rates and glucose, lactate, pyruvate, glycerol, amino acids, ketone bodies, free fatty acids, and triglyceride arteriovenous concentration differences were determined to calculate net precursor-product exchange rates across the liver, gut, and kidney. The study showed that hepatic contribution of glucose and ketone bodies and the caloric equivalents of these fuels delivered to the blood was not adversely affected by either a distal splenorenal or mesocaval shunt. In addition to these general observations, isolated findings emerged. Mesocaval shunts reversed portal venous blood and functionally converted this venous avenue into hepatic venous blood. The ability of the kidney to make a substantial net contribution of ketone bodies to the blood was also observed.

Origanum Majoranum Extract Modulates Gene Expression, Hepatic and Renal Changes in a Rat Model of Type 2 Diabetes

The present study was conducted to test the effect of Origanum Majoranum Extract (OME) of leaves on alterations induced in a model of type 2 diabetic rats. Adult male Wistar rats were fed high fat diet for 3 weeks and injected a single dose of streptozotocin (35 mg/kg) intraperitoneally to induce type 2 diabetic rats. Diabetic rats were given aqueous extract of OME in a dose of 20 mg/kg orally ...

Orexin-A Improves Hepatic Injury Following Renal Ischemia Reperfusion in Rats

Introduction: Orexins are novel neuropeptides that are localized in neurons in the lateral hypothalamus. They are implicated in a wide variety of physiological functions. Orexin peptides and receptors are found in many peripheral organs such as kidneys. It has been demonstrated that exogenous orexin-A can induce protective effects against ischemia–reperfusion injury in many organs. The goal ...

Congenital Hepatic Fibrosis: An Uncommon Cause of Chronic Renal Failure

Congenital Hepatic Fibrosis (CHF) is a rare disease that affects both the liver and kidneys.  Congenital hepatic fibrosis (CHF) is an autosomal recessive inherited malformation defined pathologically by a variable degree of periportal fibrosis and irregularly shaped proliferating bile ducts. Affected individuals also have impaired renal function, usually caused, in children and teenagers, by an...

Bone imaging demonstration of osseous lesions and hepatic uptake: Amyloid and renal dialysis effect

A 64-year-old woman, with amyloid related kidney failure (On renal dialysis) had a bone scan because of pain in the right upper femur. The 99m Tc-MDP images revealed marked hepatic uptake. In addition, there was right proximal femoral concentration of radiotracer, which corresponded to a fracture site. As an added finding, the same femor demonstrated circumferential uptake at the lower po...
